
Preparing For Your Appointment

Please fill out the new patient paperwork. All patients need to complete the Informed Consent. Adult patients also need to complete the Adult Intake form, however if the patient is a minor then complete the Parent Intake form. If assistance is needed to provide your history. Ask a family member or a close friend to attend the diagnostic interview portion of the evaluation.
The diagnostic interview takes approximately 1 hour to complete. The family member or friend is free to go after it is completed.
If possible, collect records of past neurodiagnostic testing (e.g., CT, MRI, EEG, or any other brain study) and/or results from previous neuropsychological evaluations conducted at other practices, hospitals, or institutions. These would be beneficial for the assessment process.

Please get a good night’s sleep and eat a good breakfast. The diagnostic interview can take up to an hour to complete whether it’s virtual or in-person. The testing can take 1 to 4 hours to complete. Some patients may not be able to finish the testing in a single day. There are some special evaluations that require a full day to complete and will be done in office. Patients will be made aware of this at the time of scheduling.
Do not consume any alcohol, use any recreational drugs, or take any prescribed medications including narcotics that cause drowsiness or any altered mental state, 24 hours prior to your appointment. If you are being evaluated for ADD/ADHD and are currently taking a prescribed medication for it. Please stop taking the medication 24 hours prior to the appointment as well.

Please bring a list of current medications. Also, bring any assistive aids such as glasses, hearing aids, walkers, etc.
For in-person appointments. If you have not filled out the new patient packet and/or the intake form. Please arrive 15-20 minutes early to fill them out. Virtual appointments please complete the paperwork at least 3 days prior to the appointment.
What to Expect During Your Appointment:
For the diagnostic interview, whether it’s conducted virtually or in-person will generally take up to an hour to complete. If the patient has a difficult time communicating, they are welcome to have a close friend or family member assist them. However, due to specific reasons for the evaluation this will ultimately be at the discretion of the provider conducting the interview. At the end of the interview the provider will schedule a time and date with the patient to complete the testing. The provider will also inform the patient as to whether any portion or if all of the testing can be completed virtually as well or if the patient will have to come into the office for the appointment.
For the testing, it can take between 1 to 4 hours to complete. Some patients may take longer or have to come back on a second day if they are not able to finish for any reason. If a patient needs to come back on a second day to complete testing, the provider will schedule that appointment with the patient prior to the patient leaving. Please do not leave the office without scheduling the follow-up testing date. Each evaluation will have a series of different assessments to complete some of which may be administered on a computer and others on paper. Some assessments will require one on one interaction between the patient and the examiner while others can be completed by the patient alone. The complexity of the assessments will vary from easy to difficult and will depend on the type of evaluation as to which assessments will be administered. That being said, please do not focus on how difficult an assessment may be. What is important is that the patient try their best.
Certain evaluations can take all day to complete which will require the patient to come in at 9 am in the morning in order to be completed before the end of the work day at 5pm. This is primarily due to the length of time it takes to complete the interview, the number of assessments that are administered, and the number of questions on some of the assessments. Even for these evaluations it may be necessary for a patient to come in on a second day to complete testing. This will depend on if the patient is able to work all the way through. The patient will be informed at the time of scheduling if the evaluation they require will take all day to complete. The patient will have to plan accordingly.
For all evaluations, patients will be able to take breaks when allowable to use the restroom, take a breath, get something to drink or something to snack on. We have complimentary snacks and drinks, but you are welcome to bring your own lunch or go out for lunch during a break. What to Expect After Your Appointment:
When the testing has been completed. All the assessments will be scored and the data will go to the provider for review, and along with the information from the referral and the diagnostic interview, the provider will complete the patient report. This will take on average 21 days to complete and to have your report sent to your referring providers office. Follow-up appointments are optional, but you are welcome to schedule a follow-up if you were not referred by a physician or need a more detailed explanation of the evaluation report and its recommendations.
