Forms

All patients must fill out both the “Adult Intake Form” and the “Consent Form” prior to being tested.

The Process

  • Day/Night Before Testing

    • Please try to get a goodnight sleep

    • Please bring your completed paperwork, list of current medications, and all assistive devices (this includes glasses, hearing aids, walkers, etc.) to your scheduled appointment

      • If you did not complete the paperwork, please come 15-30 minutes early to complete it in our office

  • Day of Testing

    • Please eat prior to coming to our office

    • You will have a clinical interview with one of our clinician and will be tested afterwards with one of our Psychometrists

  • After Testing

    • You will be directed by Advantage Healthcare Systems on your next steps

    • It takes 21 business days for Dr. Hopewell/HNC to send Advantage Healthcare Systems a copy of your report
