What to Know…

• Neuropsychological Evaluations (NPE), and Psychological Evaluations (PE), take between 2-5 hours on average to complete. PE’s typically take less time to complete than NPE’s. The evaluation itself will consist of a diagnostic interview and assessments. The number of assessments each patient will need to complete will largely be dependent on what the patient is being seen for. However, after conducting the diagnostic interview the provider may choose to include some additional assessments or exclude others. Some assessments will be computer based self report style while other assessments will be administered one on one. Please be aware that each assessment varies in the amount of time it takes to be administered. Overall, how long it takes to get through the full evaluation depends largely on the patient. Due to what the patient is being seen for as well as other factors such as age, mental health conditions, different mental states, cognitive abilities, medical conditions, and injuries, just to name a few, can affect how long it takes a patient to work through everything. Please be aware, if a patient shows up on the day of their evaluation under the influence of alcohol or drugs, the evaluation will have to be cancelled. Whether it is able to be rescheduled and when, will have to be determined. So please take all of this information into consideration and plan accordingly for the evaluation. • Las Evaluaciones Neuropsicológicas (NPE) y las Evaluaciones Psicológicas (EP) toman en promedio entre dos y cinco horas en completarse. Los PE normalmente tardan menos en completarse que los NPE. La evaluación en sí consistirá en una entrevista diagnóstica y evaluaciones. El número de evaluaciones que cada paciente deberá realizar dependerá en gran medida de por qué se esté atendiendo al paciente. • Patients will need to provide a copy of their driver’s license or a state issued ID card.

• Patients will need to provide a working email address to receive links for virtual appointments, and links to online assessments as part of the evaluation. Please note, all online assessments will need to be completed within 5 days from the date they were received.

You can upload a copy of your driver’s license or state issue ID when you complete the Informed Consent.
